	2		Open the file in Excel (Check Tools/Macro/Security/Medium). Click "Yes" to the macros
	3		Complete the questionnaire by clicking the boxes with your mouse
	4		Save a copy of your questionnaire to disk.
	5		Pay us securely through PayPal. Your report will be e-mailed back to you on receipt of the processing fee.
